Jay Anthony Fernandez, 63, died April 29, 2020
Born September 21, 1956 to John and Tillie Fernandez
Graduate of West High School 1975.
Jay was a life long Green Bay Packers Fan.
Survived by his mother, his twin daughters Sarah & Gabrielle Wisner (Hendersonville, TN).
And son John Fernandez. Sisters Jeanette, Juanita (Bill) and brother Larry (Joyce), and a very special friend Lana
Proceeded in death by father, a brother Anthony and a foster brother Elmer.
A Funeral Mass with be held at a later date.
Jay can finally be at peace.
